The GAIT Trial (NIH-funded): The Glucosamine/chondroitin Arthritis Intervention Trial — a large, rigorous multi-center RCT — found that the combination of glucosamine hydrochloride and chondroitin sulfate was associated with significant pain reduction in the subgroup of participants with moderate-to-severe knee osteoarthritis pain, outperforming placebo and performing comparably to celecoxib in this subgroup.

MOVES Trial and CONCEPT Study: The MOVES trial (2015) compared chondroitin sulfate to celecoxib in knee OA and found comparable reductions in pain, stiffness, and functional limitation at 6 months. The subsequent CONCEPT trial (2021), a large European RCT, similarly found pharmaceutical-grade chondroitin sulfate to be non-inferior to celecoxib for knee osteoarthritis, with a significantly better gastrointestinal safety profile.

Structural benefits: Some long-term studies (12–24 months) suggest chondroitin sulfate may slow joint space narrowing — a marker of cartilage loss — in knee osteoarthritis, though this evidence remains under ongoing investigation.

Chondroitin sulfate and glucosamine sulfate are frequently combined because they act through complementary mechanisms: glucosamine serves as a substrate for glycosaminoglycan synthesis, while chondroitin inhibits cartilage-degrading enzymes (MMPs and aggrecanases) and supports cartilage hydration. The combination may offer additive benefit for cartilage maintenance.

Quality matters significantly: Chondroitin supplements vary widely in actual chondroitin content — some products contain as little as 10–20% of labeled chondroitin. Third-party verification is essential.
