Acetylcysteine is a Dietary Supplement used as a liver antioxidant and has been studied for a number of years. Acetylcysteine is commonly used by medical doctors, and is available in hospitals in an intravenous form used to treat acetaminophen liver toxicity.

N Acetyl l Cysteine (also spelled acetylcysteine, n-acetyl cysteine, n acetylcysteine, or NAC) is a form of an amino acid. N Acetyl cysteine is a precursor in the body to the antioxidant glutathione, which is produced intracellularly, particularly by the liver.* For more information, see bottom of page.

Additional information
N acetylcysteine is an excellent source of sulfhydryl groups, and is converted in the body into metabolites capable of stimulating glutathione synthesis. Administration of n acetylcysteine has historically been as a mucolytic [mucus dissolving] agent in a variety of respiratory conditions. Acetylcysteine is often used in a hospital setting to protect the liver in cases of acetaminophen (Tylenol) overdose. In this situation, n acetylcysteine is given intravenously.*

N Acetylcysteine supplement questions:
Q. Can aceytylcysteine supplement be taken the same day as MultiVit Rx or alpha lipoic acid antioxidant?
   A. If you plan to do so, we suggest using only half a capsule of acetylcystiene, and using this nutrient only 3 or 4 times a week.

Q. Why do you suggest not taking N acetyl cysteine supplements every day?
   A. There is a rare possibility that taking N acetylcysteine every day may inhibit the body's own production of glutathione, or can form a red blood cell-derived molecule that makes blood vessels think they are not getting enough oxygen. These studies were done in rodents and high dosages of N acetyl-cysteine were used. We don't have much research in humans to determine the health benefits and risks of daily N acetylcysteine use. For these reasons, we think people can get the benefits of N acetyl cysteine by using this supplement every other day or 3 times a week and minimize or eliminate the potential side effects.

Q. What is the right n-acetyl cysteine dosage as a daily supplement?
   A. There are no accepted guidelines on the appropriate n-acetyl cysteine daily dosage. Some people may need none, others may benefit from taking 500 mg of n-acetyl cysteine two or three days a week. If you are taking other antioxidants, we suggest you reduce your dosage of n-acetyl cysteine.

Q. What is the difference between cysteine and N Acetyl cysteine ? Which one is better to take?
     A. Most often, n aceytylcysteine is the one taken since it has stronger antioxidant potential, however it depends on the condition being treated. N-Acetyl cysteine has an additional acetyl group which makes it more potent as an antioxidant.

Q. There are so many antioxidant supplements to choose from, including acetylcysteine, alpha lipoic acid, acetylcarnitine, mangosteen, acai, goji berry, pomegranate, resveratrol, lycopene, etc. Which one is the best?
   A. This is impossible to say and it is impossible to predict which one of these in which dosage would be the best choice for a person to take. One option is to buy acetylcysteine and a few other supplements and then alternate their use. One day take acetylcysteine, another day take pomegranate extract, another day resveratrol, you get the point.

Q. I plan to take acetylcysteine supplement for a long time. What is the ideal dosage?
   A. Since each person has a unique biochemistry, it is impossible to say what the ideal acetylcysteine dosage would be. One option is to take one acetylcysteine capsule twice a week if you plan to take it for many years. Another option is to open a capsule and take a third daily with a day or two off each week.

Q. I am considering ordering your N-Acetyl-L-Cysteine ( N-A-C ), capsules and have a question. Are they vegetarian? I had read that some cysteine supplements are derived from human hair, are yours?
   A. N Acetyl L Cysteine is made in the lab and it is not derived from animal sources. However the gelatin capsule is not vegetarian.

Since this supplement is beneficial to liver health, and so is silymarin derived from milk thistle, are there any safety concerns when combining them?
Not that we are aware of, but you may also consider alternating their use.

Study
A retrospective evaluation of shortened-duration oral N-acetylcysteine for the treatment of acetaminophen poisoning.
J Med Toxicol. 2009 Dec; Betten DP, Burner EE, Thomas SC, Tomaszewski C, Clark RF.Department of Emergency Medicine, Sparrow Hospital, Michigan State University College of Human Medicine, East Lansing, MI 48912, USA.
The use of less than the traditional 72-hour course of oral N-acetylcysteine has been an alternative treatment option following potentially toxic acute and chronic acetaminophen ingestions felt to be at low risk of developing hepatotoxicity. While clinical experience with shortened treatment duration is extensive, there are few studies evaluating the effectiveness and extent to which these regimens may be used. A large statewide poison center database was reviewed for all acetaminophen exposures involving potentially toxic acute and chronic ingestions, in addition to those taking place at unknown times. Patients were identified who met laboratory criteria for early N-acetylcysteine (NAC) discontinuation after a minimum of a 140-mg/kg oral NAC loading dose and 5 additional 70-mg/kg doses over 20 hours. A further search of the poison center database was conducted for individuals who received shortened-course (20-48 hours) oral NAC treatment who developed subsequent hepatotoxicity or death. Treatment with shortened-course oral NAC in patients meeting criteria for early discontinuation may be an effective treatment option in a sizeable proportion of individuals with potentially toxic acetaminophen ingestions.
